Atrial fibrillation is significant. So, there is a risk of stroke for which you have been using blood thinners. Supraventricular tachycardia is significant too.

The following are treatment options of supraventricular tachycardia:
Calcium Channel blockers
Beta blockers
Dioxin.
Cardioversion
Catheter ablation
Important thing is patients with symptoms of Dizziness should not take these medications as they further decrease the blood pressure.

Coming to pauses, a pause with duration of more than 3 seconds during day time is significant. The following are causes of pauses :
Sinus node dysfunction
Obstructive sleep apnea
High dose beta blocker therapy

As your pauses are significant, it's better not to use beta blockers. Remaining medications are to be continued. Please do consult a cardiologist for catheter ablation therapy for supraventricular tachycardia. Cardiologist will decide if you require any pacemaker as the pauses are significant.
