Brief Answer:
Anal fissure highly likely

Detailed Answer:
Hi,

There are number of cause for difficult bowel movement with blood.
The No 1 cause is always anal fissure which bleeds with hard stools. It resolves spontaneously. However in our clinical practice we always advise an elderly who crossed 60 yrs to get a anoscopy done first. This is a simple one minute procedure with speculum to view the anal tract and detect hemorrhoids, fissures.

So it is good that you had fixed an appointment. Observe the next day morning stools as well. Also take stool softner like DULCOEASE syrup which is available over the counter. If the stool softens observe if the blood still comes with stool or not.
