Welcome to Health Care Magic
LDL (& non-HDL) is bad cholesterol – lower, the better.
HDL is good cholesterol – higher, the better.
LDL value of 203 is very high. In the absence of any other risk factor 100 is ideal. [130 was the limit before]. In the presence of ischemia, 70 should be the aim.
TG (Triglyceride) also is high
Your 3–step approach is proper.
Exercise – walking 1 hour is good.
Diet – you say is controlled....
Drug – Statin, you are already taking.
Probably, there is a strong genetic basis, leading to this high level and not coming down.
Since the TG is also high, you can add Fibrate – Fenofibrate 160 mg (micronised) will be the first choice. You can get it separately or in combination with atorvastatin. The combination can cause or worsen (calf) muscle pain – Beware. Hopefully, it will come down.
Repeat in 6 weeks (Strict 14-hour fasting is necessary for TG). If LDL is still high, step up atorvastatin to 40 mg. (The maximum dose of atorvastatin is 80 mg.). Most likely, it will settle down
Repeat in 6 weeks again – if still high add Ezetimibe 10 mg. All will be well
Take care
Wishing you speedy recovery
God bless
Good luck
