What Should Be Course Of Medication For Iron Deficiency Anemia?

Question: I don't feel my doctor is appropriately treating my iron deficiency anemia. I would like someone to take a look at my bloodwork and see what recommendations they may have. I had one 200 dose of venofer but then no more. My hemoglobin and hematocrit remain low. My ferratin did increase from 5 to 20. My iron and iron saturation are still low. I do not believe my doctor gave me the correct dose of venofer. My current levels after one 200 venofer infusion are as follows: hemoglobin 10.6, hematocrit 36.5, Ferratin 20, transferrin 277, iron 18, iron binding capacity 368, and iron saturation 5%. I cannot take oral iron. Previous gastric sleeve surgery, heavy menstrual periods.

Detailed Answer:
Hello , Normal hematocrit in females ranges from 36-48%. Your hematocrit after injection is 36.5 which is good. Ferritin indicates stored iron in the body. As it has increased from 5 to 20, we can venofer as effective enough to increase stored iron in the body.
There is another preparation called ferric carboxymaltose. It is marketed as Injectafer. It is well tolerated and less hypersensitivity reactions than venofer.
Administration :
≥50 kg
750 mg IV in 2 doses separated by at least 7 days; not to exceed cumulative dose of 1500 mg per course
Alternatively, may administer 15 mg/kg IV as a single dose; not to exceed 1000 mg
<50 kg
15 mg/kg IV in 2 doses separated by at least 7 days.
Iron values must be repeated after 1 and 3 months of giving the injection. You can take another dose of venofer after 1 month or take Injectafer instead.
Please don't worry as your values as per your reports are not bad and they are improving after venofer injection. You can repeat venofer or start Injectafer after discussing with your physician.
