4.5 mm is not much bigger. It depends where is the stone located, causing any obstruction or not, any infection in urinary system and so many factors that decide the management.
Small stones usually get disintegrated in some patients, or pass done the ureter, bladder and urethra, may be with pain. It can cause problems only if the stone is causing any obstruction.
