What Symptoms Are Indicative Of Kidney Stones?
Question: I have continued unilateral and bilateral flank pain, intermittent with spitting up, in addition to pain under my rib cage. The episodes have come on over a few days, and some have come on quickly. On a 1 to 10 scale, 10 being the worst... it usually stays at a 5 and has moments of a scale of 10 that can last moments or 1/2 hour stints. I did go to my primary who sent me for an ultrasound that came back normal.
These episodes have been going on for 7 months. I also have been getting up to urinate 1 to 2 times per night, and always feel thirsty (I do do not have diabetes). I have always had blood in my urine since 1996. I have cast in urine, and even immature granulates in my blood. In September of 2017, I had a erg reading of <40. However, it comes and goes.
I have a pending visit with my Gastroenterology for mid March. My Maternal Great Grandmother had kidney disease, my Maternal Grandmother found out she had a shriveled up kidney when she was in her 80's. I have had multiple kidney infections in my youth and passed a stone during pregnancy 17 years ago.
Can you list any possible kidney issues that can be occurring that would give episode type symptoms and readings and pain, and that would not able to be discovered using ultrasound?

The pain is severe, spasmodic; flanks and below ribs, intermittent with spitting/sitting up - for 7 months - always thirsty - noted all other details also.
Since ultrasonography of abdomen is normal any gross abnormality is ruled out. And thus the proper clinical evaluation, examination and necessary advanced investigations like CT scan of abdomen along with Enteroclysis for intestines, IVP that is intravenous pyelography for urinary system and an MRI of the spine for any neurological connection can help to get the proper diagnosis.
To answer your question in specific: Problems in ureter only can give such symptoms but then the ultrasonography should have shown some changes in urinary tract; hence the need for proper evaluation as discussed.
Also needed is Detailed investigations of blood, stool, urine as per the clinical findings of various Specialists and coordination among all these.
This way alone a proper diagnosis can be done for proper management.
