Hi,

All the findings indicate that there is pressure on your (R) median nerve and all other nerves are normal.

In my clinical practice, if you have symptoms suggestive of carpal tunnel syndrome( pressure on the median nerve at wrist), I would take you up for surgery.

The severity depends on your symptoms. If you are not able to sleep well or have pain in your hand most of the time then you should go for surgery. Reports convey that there is a problem (significant) but surgery will depend on your symptoms.
