untreated Lyme disease may cause chronic symptoms. The term "chronic Lyme" is not quite right though because in many cases the pathogen itself is not present! When there is enough evidence of the presence of Borellia (the causative agent) then antibiotic treatment is administered. For example in a patient with joint pain, fluid aspiration from the joint capsule may identify signs of inflammation. In such cases antibiotics are given and the patient may improve. Some patients never improve or improve only partially probably because changes in body tissues have occurred due to the inflammatory processes that can't be reversed by antibiotic treatment and eradication of the pathogen.
