When I Leave The House And Go On Public Transportation

default
Posted on Wed, 28 Nov 2018
Question: When I leave the house and go on public transportation such as train or bus I start to worry that I’m going to vomit or get sick from someone else or think I have some illness. And I’d start to feel a little nauseous (like loss of appetite) not major nausea, and my heart will start to beat a bit fast and I’ll just feel a hot feeling but then it goes away after a few minutes if I smoke or start walking around doing something or I’ll have a sip of water, listen to music or keep myself busy. Also, I googled symptoms and some of the conditions google lists are serious conditions. I start to get scared and don’t want to go places because I think I’m going to vomit, and I don’t eat out in public because of the same reason above. And I’m afraid of getting full because maybe I’d throw up or something and then start feeling the same way, so I’ll smoke a cigarette and I’ll calm down. I haven’t been eating much (I’ve went 1-2 days without eating out of fear) and I just drink water and eat little because I don’t want to feel sick. I also check the food like 100% to make sure it’s cooked and who ever cooked it washed their hands, if the food is old, or has an odd taste or someone else gets sick from it then I start to think I am. There were days I thought I was sick and just stayed in bed all day. also when I’m out in public eating or just in a place outside my home then I’m like feeling my forehead, neck and back constantly checking myself if I have a temperature. This has been going on for about 7-8 months. What’s causing my symptoms? Do you think It’s some type of underlying illness?? I google a lot and sometimes makes things worse.

Brief Answer:
Symptoms are concerning for panic disorder (specifically Agoraphobia)

Detailed Answer:
Hello,
Welcome to Ask a Doctor Service.

I have gone through your query in detail and I can understand your concern.

Since you are experiencing the symptoms only when going outside or in public places, the most likely consideration is underlying psychological factors contributing to the symptoms.
Feeling of getting sick and anxiety while in public places and trying to avoid situations that trigger the symptoms could be due to Agoraphobia.

Detailed evaluation by Psychologist/psychiatrist is required to confirm the diagnosis.

If the diagnosis is confirmed, management will be muti strategic including anti-anxiety medication, cognitive behavioral therapy, and psychotherapy.
I can understand that you are going through a difficult situation. However, with appropriate management, it is completely possible to achieve symptom relief.

So, please be advised to follow up with a psychologist/psychiatrist.

Hope I was able to help you

Yes most of the time it’s when I’m out in public places I feel this. Very rarely do I feel it when I’m in my room, the times that I do feel it in my room are when I just come back home and I need calm down in my room or after a cigarette. And if I think about it then I experience symptoms

Brief Answer:
Advice to follow up with a psychologist

Detailed Answer:
Hi again,

The symprtoms are very typical of anxiety/panic disorder (agoraphobia).
It should also be noted that you are trying to break the anxiety episode by smoking. However, smoking also has its own harmful effects and it can be very difficult for you to break the habit on your own at this point.
Appropruate management under a psychologist/psychiatrist can help manage both the anxiety symptoms and help you quit smoking too.

Please let me know if you have any more questions
