When During The Day Should Losartan And Tamsulosin Be Taken?

Question: My father takes Losartan & Tamsulosin. What would be the best times to take each of these? He currently takes tamsulosin at night, but still gets up 4-5 times per night.

Hello,

Based on the facts that you have posted your father who is 92 years old has obstructive voiding symptoms due like frequency for urination ,nocturnal frequency (Nocturia) due to enlarged prostate and have been taking Tomsulosin and Losartan for hypertension .However he still has to wake up 4 times during night .

Tomsulosin is the drug the relaxes smooth muscles in bladder neck and prostate and helps to improve the urine flow but has no action on irritability of bladder muscle .It has action for 24 hours and there would not be any change in action as for as timing of taking the drug is concerned .

Losartan should be taken early in the morning to keep the Blood Pressure under control through out the day .

As regards his Nocturia (4-5 times ) in night .It needs to be reevaluated about the size of the prostate and Post Void residual urine (PVR).

I would suggest you to consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis with specific mention of Post Void Residual Urine .
3) Renal function tests
4) Cardiac status evaluation
5) Basic blood tests for diabetes and liver function .

If he is in a good state of general health I would suggest to discuss with your Urologist about exploring a possibility of endoscopic surgical treatment as an option to remove the prostate and to make a passage free from obstruction .This will make him free from obstructive voiding symptoms and to improve his quality of life .
