Whether I Need To Continue The Ointment Repeatedly Or Any Other Suggestion Is There?

Question: I have inflammation like spots on my penis glans(portion below foreskin). Earlier it was confined to a small portion and now it got spread to majority of that area. I dont have any itching or noticeable pains. Doctor identified this to be Zoon's balanitis some months back I have been prescribed with mometasone furoate 0.1 % ointment for a few days. I applied for around 10 days and got almost full recovery and so I stopped using it. After a few days, I could see that it reappeared again and now stays. Can you please tell me whether I need to continue the ointment repeatedly or any other suggestion is there? It is concerning me since it is not getting cured completely also having concern if I can use this ointment continuously.

You had not mentioned your age, marital status, your sexual activities, whether you are a diabetic or not. These details will help for further discussion. You can upload the picture of the lesions.
Plasma cell balanitis of Zoon will get better with circumcision. Repeated frequent cleaning of glans penis and the coronal sulcus with plain water (avoid soaps) will also help you to become better. Retract the foreskin completely without any wrinkles, expose the glans and the sulcus and wash using water and your fingers.

Hello Doctor,
Sorry for not being informative with my question. I am 40 years old and married. I do not have diabetics. I think this problem appeared within the last 2 to 3 years. For the past 4 years, I have sexual contact with my wife using normal latex condoms only prior to that I had sex without condoms with her. I have attached a couple of images also for your reference. Please let me know your recommendations considering the above information as well. If you want to know more information, please let me know. Let me know if it will get cured with frequent cleaning and drying or if it is advisable to continue using the ointment mometasone furoate or any other ointment or any further medications.

Hi,
As your doctor mentioned, it seems to be Plasma cell balanitis of ZOON. Only the biopsy can confirm the diagnosis.
You cannot apply Mometasone for longterm applications as steroids have health hazrds.
If possible avoid using condoms. It could be due to irritation to the latex rubber (possible with some individuals).
Wash your glans and sulcus 5 times daily with plain water and look for improvement. If you feel better continue the habit of frequent washing. (In places where you cannot get good clean water you can wash with your urine itself by holding your foreskin stretched and closed and try to pass urine, then open and let out the urine. You can repeat the procedure 3 times during every act). If there is no improvement then opt for circumcision.

Hello Doctor,

Thank you for your advice. I will try to wash frequently as per your suggestion and see for results. Should I need to wipe out after wash or is it fine to leave as such? One problem I missed to mention in my previous update is that I do have dry eczema in my legs. I'm not sure if it has anything to do with this. I used to just apply Moisturex cream for that. Regarding the usage of condom, I use it since I do not want to get a pregnancy and I don't know any other approach. Is there another better way to avoid pregnancy? I can understand that, circumcision seems to be the last resort to get relief, which i think is going to be a surgery. In the worst case, is there any harm if I do not go for a cirumcision?(what I mean is will there be any problems arise if it is untreated or any side effects)

Hi,
Welcome back.
After wash with water, it is not mandatory to dry the glans penis.
Eczema has nothing to do with your present problem over the genitals.
There are many otherways for contraception like oral pills, intrauterine devices and long acting injectables etc. as per the needs of the individual couple (duration of contraception in years, health conditions where in some cases oral pills contraindicated). You consult with your physician they will direct you.
When you are having children the best thing in my opinion is permanant contraception (vasectomy for men, tubal ligation for women), that too vasectomy is the best. You need not get into the abdomen, very minor surgery, you can resume your norms after the surgey. It will no way affect your sex life, you will have your ejaculations (except the sperms). Really you will feel better after the vasectomy, can have sex without any fear of pregnancy. If the balanitis is left untreated for years (say 10-15 years) very very rare chances for this balanitis may go for maliganancy.
