Who Should I See Urologist , Gynaecologist Or Vascular? CT

Question: Who should I see Urologist, Gynaecologist or Vascular? CT scan shows varices left ovary and varices left kidney vein branches with renal calculi (small calculi)
Had 2 dilation of urethra in past, ongoing urine and kidney infections, some incontinence. Increased IBS symptoms, Constant lower painful pelvic pain for at least 3yrs or more, back pain left side, pain after sexual intercourse. Seen Urologist who tried to give Amitriptyline (which can't take previously tried through previous Urologist caused retention) didn't seem bothered by varices said refer to pain clinic and Gynaecologist

Brief Answer:
Visit a vascular surgeon.
Detailed Answer:
Hello dear, welcome to Ask a doctor's service.
I read your query and here is my advice.
I think you should visit a vascular surgeon to talk about varicose veins surgery.
I think the varicose veins in left ovary and kidney may be causing the pelvic pain and IBS symptoms.
I don't think you need to visit a pain doctor or urologist neither a gynaecologist.
You may take an opinion from a gynaecologist about reproduction issue.
So varicose veins are the cause of your pain and surgery may be a solution.
Hope I have answered the question.
