Question: Hi i have visible blood in my urine and have struggled with an infection since mid Alril. I am currently taking my third dose of tmp-ds or sulfamethoxazole. No great pain, a little burning upon discharge. No bacterial growth in last culture. I have an appt. with an urologist but I'm concerned with the bleeding. what are my next steps? ER or wait?

Hello,

Based on the facts that you have posted the blood in urine that you are passing is mostly due to Enlarged Prostate .However other cause for hematuria has to be ruled out by relevant investigations.

I shall be happy if you can post me more details to help me in making precise comments
1) Do you have other obstructive voiding symptoms like frequency ,urgency for urination ,poor urine flow etc?
2) Is the hematuria persistent and gross or occasional with few drops of blood?
3) Are you taking any blood thinner medication like Aspirin etc?

The hematuria at this age is mostly from the a small vein on the surface of enlarged prostate which gets teared off causing hematuria and seals of spontaneously and is not usually alarming to need urgent intervention .You can certainly wait for few days till you get appointment with Urologist .

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Endoscopic Examination of bladder (Cystoscopy)

Further treatment will depend upon result of these tests and final diagnosis.

In the mean while start taking antibiotic like Cefixime and Nitrofurantoin twice daily along with Tomsulosin once daily .

Get the prescription of medicines from your family Physician.

Ensure to drink more water .

1) and 2) do have frequency issues, some urgency to urinate, little pain. The blood has gotten slightly worse in the urine. definitely not a few drops quite a bit of blood. I have Afib and I take Xarelto 20 mg. I also take cardizem la 120 mg, metoprolol 50 mg, lisinopril 30 mg, simvastatin 10 mg and janumet 50/1000.

Hello,

You probably seem to have Enlarged prostate with he matures triggered by side effect of drug that you are taking for Atrial Fibrillatio.

Stoo taking Xarinto for few days and get investigated to rule out Enlarged Prostate.
