
Why Everyday I Wake Up With A Very Severe Right Jaw ?

hi i have well controlled hcom . heart rate runs from 70 to 105 with excercise taking metroprolol 37.5 mg daily , no dm, or other major issues
just went to er with covid symptoms and they tooke ekg (cause of hcom dx not due to heart issues i stated, it showed sinus rhythm and interior mi. i had and ekg in february that was basically normal sinus rhythm hr 57, ( took 50 mg of metroprolol that day)
and otherwise normal .... i did wake up with a very severe right jaw pain about a month and a half ago . i am a nurse practitioner

I passed carefully through your medical history and would like to explain that considering your recent symptomatology (severe pain) and ECG changes it is necessary to further investigate on the exact possible underlying reason.
As you are diagnosed with HCOM and actually cardiac ischemia is a well known finding in such patients due to inappropriate myocardial mass increase (not associated with adequate micro-vascular tree supply), it is necessary to study such an alternative potential physio-pathological pathway.
In addition, a potential inflammatory myocardial involvement (myocarditis) is not rarely encountered in COVID or other viral (infectious) clinical conditions.
Coming to this point, tissue characterization by means of cardiac magnetic resonance would be the most appropriate test to investigate and differentiate between possible inflammation or myocardial fibrosis due to ischemia or increased myocardial stress.
