Brief Answer:
Cervical spine chronic changes most likely cause

Detailed Answer:

Hi,

Judging from that description you provide the most likely cause would be chronic neck degenerative changes, common with age, which are commonly associated with muscle stiffness and increased muscle tension which can irradiate in the head causing associated headache. Tylenol is not very effective in relieving symptoms. Over the counter anti-inflammatory medication like Ibuprofen or Naproxen are usually more effective. Using such medication for a long time can have its side effects though so for long term benefit some physical therapy may be considered as well.

Another possible cause to be considered would be high blood pressure which may cause headache which may irradiate in the back of the head. So if not done already blood pressure monitoring for 7-10 days would be recommended as well.
