Question: I will start out with a little background information, then gradually lead up to the present day situation. I am 5'8, 144lbs. ~27.75 yrs old. ***I would like a urologists' opinion***. I've been working out pretty regularly, although lately I may have worked out too much. I will explain below. As a forewarning, I wanted to be thorough and not leave out any detail so as to get the most informed response(s) possible, so this query will be rather lengthy.
A little over 3.5 years ago(Sept. 2010), when I was 24 yrs old, I had a
kidney infection, which regressed into a UTI. I took antibiotics prescribed by a doctor for a few days, and it went away. However, not too long after, every now and then I would have lower-left abdomen and lower-left back "pain spikes". Happens more in the abdomen. Anyways, these pain spikes didn't happen very often, and there was no urinary or sexual problems, so I didn't go back to the doctor.
My right testicle is always harder and larger than my left; however I don't think it is abnormally larger/harder. Here's why I am mentioning this: when my
scrotum is fully relaxed, i.e. "at maximum droopiness", I can sometimes see what looks like a fairly short, skinny blue vein in my right testicle. I can feel when anything touches my right testicle, and it ascends/descends normally for temperature adjustments, no bumps on the testicle, but it seems like there is never anything going on in my right testicle or sperm cord. What I am getting at is, sometimes when I ejaculate, it's like I can feel sperm being drawn from my left testicle, and my left testicle/sperm cord feels dull and empty post ejaculation. I don't know if that's normal, but that feeling is not in my right testicle...as if nothing is "being transported", or happening, ejaculation or not. Maybe I am "feeling more" in my left side because my left side is weaker, since I am right handed? Just a guess.
It just seems like both testicles/cords would have a "something's happening" feeling, especially during or after ejaculation, or especially if you masturbate regularly like I do. It just seems as if there is no sperm production/sending/activity going on with my right testicle, although there are no lumps or pains on the right side. I also get an empty/dull feeling in my left sperm cord/testicle post ejaculation, again, no strange feeling in my right testicle/cord.
I don't remember having this "empty" feeling on my left scrotum before my kidney/UTI incident.
I think I should also mention I had been doing a lot of push-ups every day for about a week, and the last day I did this "pushup regiment" was either 4/6 or 4/7(2014). Also, about a week before than, i had been jogging but stopped because of ITB problem in right knee. Although ITB pain is more than likely unrelated to my current problem, I just wanted to mention that I had been jogging somewhat regularly before my pain/current problem.
It started out as a heavy-ish, achy feeling in my right testicle, slightly more heavy than usual. There was a slight "bump' above my right testicle, but it could have just been the
epididymis head filling up from "edging". I had "edged" the day before, where I masturbated to orgasm, but didn't orgasm by holding it off through sheer force of will. I did this so I would have a bigger load next time I masturbated. I hadn't edged in a few years, so maybe that's the problem, I dunno. I may've waiting to long to ejaculate after edging. Then, I went to sleep, had to get up in middle of night to pee, afterwards decided to masturbate to orgasm, which was a fairly big load, then went back to sleep. When I woke up 3-4 hrs later, I noticed that, not only had the "bump" above my right testicle had gone away, a dull/empty feeling in left sperm cord+testicle. I stupidly masturbated to orgasm again; my dull/empty feeling then seemed to become more noticeable, and worst of all, I also began to have pain behind the upper-left side of scrotum, where scrotum connects to groin.
This pain was somewhat intense, depending on how much I moved my left leg. I could walk around, but very slowly. I could not lift my left leg very high without getting more pain. This lasted all day. The next day, there wasn't any, or nearly as much, but it turned into the fluttery, spasm-like feeling, and the empty-ish dull feeling in my left sperm cord//testicle lessened, but was still present. I remember lifting my left leg slowly, then it hurting when i lifted it all the way. I have also woke up in the middle of the night, with the bottom left testicle hurting near the back side for about a second or two. I then pee for a normal amount of time, no funny smell or funny looks or pain, then go back to sleep. I think this is happening possibly because I am taking saw palmetto to see if that will alleviate my fluttery left groin area.
The next day, the pain had gone away altogether, but the fluttering/intermittently-spastic, and basically always uncomfortable feeling began happening in the area above my front upper-left scrotal area.
Currently(4/15), I get a weird feeling in what feels like my left sperm cord when I lift my scrotum towards my body, so I've been leaning more towards the problem being with my scrotal veins/cord. I did some research and thought I -might- have a
varicocele. The only problem with this is this my scrotum does not look or feel like a bag of worms. I currently don't have any problems urinating or getting erections; I still produce sperm, or at least ejaculate fluid. I have no bums/lumps on my testicles, scrotum, groin or lower abdomen. When my testicles/scrotum are relaxed/sagging, I will sometimes feel and notice that the front of my left sperm cord is slightly rigid or bumpy.
It has been 5 days since the pain, I have not tried to orgasm since that seems to be a possible trigger of the pain/empty/dull/fluttering that's happened. Please note, I am currently not in any pain, even when I lift my left leg. It is really just an uncomfortable, annoying feeling in my upper left scrotal area, maybe
slightly above left side of scrotum, maybe in my left sperm cord. It seems to be lessening, but not as fast as I'd like it to. Last night, I did not get up in the middle of the night and pee, or wake up with scrotum pain at bottom left of testicle. I think it's because I didn't take any saw palmetto yesterday(4/14).
So, in short, I thought I had a varicocele at first, but then I thought it was something else because I don't look or feel like I have a bag of worms. I'm thinking I may have over-strained some muscles on my left scrotum/cord/testicle. Any help/advice will be greatly appreciated, and thank you for your time/effort.
