Why Mole Gradually Change And Then Have A Much Raised Appearance?

Question: Hello,
Around 6 weeks ago a mole on my back started to itch and there was some pain (I originally thought it was irritated by scented lotion wash). I tried to avoid scratching it, however I did once and a scab formed within a few weeks since it originally itched. It is in a place where it can easily get caught on clothing.
The itching stopped and then it felt better and it only felt tender if it would get caught, etc. However weeks later, I noticed it was raised and it seems to appear to have a stalk like appearance (I am also wondering if a piece has fallen off). I am attaching some photos for your review. You will notice the mole gradually change and then have a much raised appearance. The newest photo with the stalk like appearance is highlighted using a torch so it appears darker around the skin due to a shadow.
