Why Occasional Premature Ventricular Contractions?
My recent test results in April 2022 are as follows:
1. Total cholesterol - 155 mg/dl, LDL 95.6mg/dl, Triglycerides 92 mg/dl. FBS 90 mg/dl, Hb1AC 5.7
2. Sodium 140 mmol/L, Potassium 3.92 mmol/L, Uric Acid 5.9 mg/dl, Calcium 9.3 mg/dl, Phosphorous 3.6 mg/dl
3.Serum Creatinine 1.22 mg/dl (lab range 0.72-1.20 mg/dl).
4. BUN (blood urea nitrogen) -11.9 mg/dl
5. Thyroid tests - Free T3 - 3.34 ng/dl, Free T4 - 1.58 n/dl, TSH 0.76 uIU/ml
I attach my latest ECG report of August 2022 and my previous ECG in July 2021. My queries are as follows:
1. The report mentions Sinus Tachycardia. I was anxious during the checkup (even in July 2021) and noticed my increased heart beats/ chest thumping. My usual pulse rate is 60 - 75 bpm, as I exercise daily. I have never had any episodes of tachycardia / palpitations on a regular basis. During this ECG I took several deep breaths to control my heart beats and the technician adjusted the leads a few times while asking me to relax.
2. Non specific T wave abnormality -please explain what this means. The ECG in July 2021 mentioned "non specific ST wave abnormality". I did an Echo in July 2019 and the result was normal with EF 74%.No episodes of chest pain, fainting, etc.
3. The ECG mentions " occasional Premature ventricular Contractions" - was this due to my anxious state and fast pulse rate? I have never had any episode of fainting, feeling of missing a heart beat, etc.
3. Please advise if I need to take any further action.

After going through medical query I can understand your concern and I would like to tell you that the blood reports which you have mentioned or fairly alright and there is nothing alarming about them.
As you are a nephropathy patient so the creatinine level which your blood reports are showing as of now is acceptable. Continue treatment as ongoing for the same.
Now answering your questions
1. As you exercise regularly and your usuals heart rate is well under control, the tachycardia which you had during ECG is only bcoz of anxiety.
2. Non specific T waves, as the name suggests are non specific and cannot be allocated to any specific cause. As your echo was alright, there is nothing to worry.
3. Regarding premature complexes, I would like to have a look on your ecg. Kindly upload a good quality image of your ecg for review.

I have reviewed both the ecg and I would like to tell you that these are occasional ectopic in ecg and do not require any medical attention.
However if you feel them quite too often throughout the day or if they are affecting your quality of life then a HOLTER monitoring is advisable to understand the total ectopic burden in 24 hrs.

1. I was quite anxious during the ecg and took deep breaths to calm myself. I have never experienced any PVC in day to day life. Is it possible that the PVC was due to high anxiety occurring at the ecg ?
2. What do you mean by ectopic?
3. The ECG in July 21 stated " non specific ST abnormality" and the ECG in Aug 22 stated " non specific T wave abnormality" - is this due to how the leads are placed ?
4. My last echo cardiogram was in July 2019 which was normal with effusion rate at 74 %. Given that I have done ECG tests in July 21 and Aug 22, when should I take the next echo cardiogram test ?
