Question: Wife 70, good health, taking low dose levothyroxine 1x/day is only med. While driving this am, she complained of 'WHIRRLIES" (unusual for her), gradually began felling weak, fatigued, nausea developed, to vomiting and some diarrhea. About 2hrs. into this. Pulse 72, steady, regular, BP 138/71, temp94.7 but she took it out of her mouth before 3 min. Sensitive to touch, talking to her, currently, lying on our bed nude (due to earlier complaint of feeling hot). Resting comfortably--no pain/cramps Felt she'd pass out early after vomiting but did not and no complaints since. Has taken in some water but small amount. Does not want emergency services. I'm watching closely and will initiate emergency contact as indicated. Thank you

Hello,

I have gone through your query and here is my advice.

As per your history, it may be gastroenteritis. So kindly follow up with a bit more information.

Please let me know if she has history of motion sickness, abdominal pain or had any outside food?

For diarrhea she can take tab. Loperamide 4 mg and repeat 2 mg after each loose stool up to maximum 16 mg per day. For vomiting kindly give tab. Ondensetron 4 mg. These medications should be available over the counter.

Along with that keep her hydrated by electral XXXXXXX (oral rehydration solution). You can give tab. acetaminophen (Tylenol) in case of temperature more than 99F.

For further assessment, I would suggest you to do complete hemogram, ultrasound abdomen, thyroid profile, stool examination after detailed medical examination and physician's opinion.

At present she looks vitally stable. Kindly do blood pressure monitoring and watch for symptoms like dizziness, rapid heart rate, fever. In that case she may require immediate medical care.

Thank you, doctor. I had her taken to the ER where they determined she had positional vertigo and dehydration. She was Rx;d meds for nausea and vomiting as needed. It's been 24 hrs since those symptoms and is considraby improved, just somewhat fatigued. She hd a CT scan and blood work up, both of which were not significant for further issues.
