Question: Will you please interpret this radiology report from my Dad's CT of the brain and advise as to possible causes?

CT scan of the brain waa done without contrast.
There is mild cerebral and cerebellar atrophy. Ventricles are mildly dilated from atrophy. There is mild decreased density in the periventricular white matter from mild chronic microvascular changes. There is mild rnucosal thickening in the right ethmoid and right frontal sinuses.
Impression:
1.     Mild atrophy and mild chronic white matter changes.

I have read your query and here is my advice.

Those CT scan findings are, in most of the cases, common findings at your father's age.

Mild atrophy refers to brain matter shrinking as we age, so, are the mildly enlarged ventricles.

Mild microvascular white matter changes are most likely caused by unstable blood pressure.

Regarding the CT report, there are no brain lesions such stroke, tumors, etc. that may be considered dangerous.

It depends on the symptoms, however, in cases of atrophy there is actually a ventricular enlargement.

I don't think that mucosal thickening of the sinuses found on the CT is an important condition.

In both cases, a correlation between symptoms and CT scan findings is necessary for a correct diagnosis.

What are your thoughts on the mucosal thickening in the sinuses?

He has experienced unsteadiness for the last few years, and over the last few months there have been cognitive changes but he still lives independently, and most recently he has had increased urinary urgency. I read this is the triad for normal pressure hydrocephalus. So, when I saw he had the mildly enlarged ventricles, I wondered if he should be tested for NPH - what do you recommend? He is scheduled for a full knee replacement this coming week, and, I don't want there to be any unaddressed brain issues that will create complications.

It is true that this triad of symptoms points towards NPH.

Removal of cerebrospinal fluid through lumbar puncture is necessary to understand correctly the condition.

If there will be significant improvement in symptoms after removing approximately 40 cc of cerebrospinal fluid then NPH is very likely the diagnosis.

He had an MRI of his cervical spine done and they found fluid accumulation in both shoulders. What would cause fluid accumulation in the shoulders? Any chance he could have NPH
