Would An Old Mononucleosis Infection Induce Ds DNA Antibodies?

Question: Hello doctor . Could you tell me if an anti dS DNA test could be slightly elevated (a 10) to be exact in conditions other than lupus? I read that mono could sometimes do that. Would an old mono infection be capable of leaving that antibody.Thank you for your time

Well, DsDNA is quite specific for lupus, but as we know it can be falsely induced by few other diseases or drugs. You are right in saying infection mononucleosis can cause DsDNa, infact any viral infection can induce the antibody along with ANA positivity as well which goes off over time. The titre of 10 is a bit too low to be significant.

The other thing which can induce DsDNa is the drugs which are of usual routine.

But all this is more significant along with clinical features coming up. It is always advisable to get a urine examination, Complement C3 and C4 along with REnal function tests if DsDNa comes positive

So an old mono infection would not leave this antibody?by old i mean years ago. Also what would levels of this antibody be in someone who has lupus typically.Thank you

Well, infection does not leave it positive for that long of a time. It is mostly an acute process which goes off with time.

The antibody levels have to co relate with the clinical symptoms of lupus like fever, hair fall, oral ulcers, joint pains and ANA positivity. If the symptoms are not present and just DsDNa is positive , it does not hold much significance as these antibodies can be falsely positive as well. Level of more than 15-20 On ENA scale holds good.

It is a very tough call but without clinical symptoms we normally dont treat the antibodies
