Would The Biopsy Need To Refer Closely To The MRI Images Already Obtained?
Question: Hi,
My wife appears to have an issue with right orbit inflammation detected on an MRI scan leading to right optic neuritis which started to affect vision about three months ago. I have attached a copy of the MRI report. The blood tests for neuro-immune disorders are negative although MS can't be tested by blood tests it is considered to be very unlikely and has effectively been ruled out.
We have seen an ophthalmologist who has suggested possibly a tissue biopsy through the lacrimal gland to determine cause.
Based on the description in the MRI report does this look like a promising course of action? I am unsure if the lacrimal gland would contain the affected tissue sample to allow a diagnosis. Would the biopsy need to refer closely to the MRI images already obtained?
Are there many lacrimal glands that would be typically have to be biopsied and are there any risks typically associated with the procedure?

Hello thanks for the question. In my opinion a tissue biopsy would help to arrive at a diagnosis. for sarcoidosis ACE levels and blood and urine samples will also help

We have just received the visual evoked potentials test results. I am not sure if these results (attached) point towards any other possible lines of investigation?

The reports suggest low amplitude which suggests axonal disease but it is not that low. But to arrive at a conclusive diagnosis we need a biopsy if not lacrimal gland then instead can we consult a neurosurgeon who can take a biopsy of the infiltrated tissue . Hope my answer helps you .
