Yesterday Morning, I Woke Up From Pain In My Testicles

Question: Yesterday morning, I woke up from pain in my testicles that reminded me of 'blue balls' and pain in my lower abdomen, right underneath my belly where the groin starts. The pain in my testicles subsided after around 4 hours, while the pain in my abdomen persisted throughout the day, and would gradually become intermittent but stay for around 30 minutes. For most of today I have experienced the same abdomen pain, though diminished in intensity, and have felt OK for the past few hours now. It has been about 30 hours since I first felt pain in my testicles. I masturbated yesterday about 8 hours after first experiencing pain, and earlier this morning, and produced significantly less ejaculate then normal. I masturbated about an hour ago and produced a normal amount of ejaculate for me. I am reading all sorts of horrible things online about hernias and testicle tortion. What should I do?

Thanks for your query and an elucidate history.
My thoughts after reading the history:
It has already been 30 hour and now the pain is intermittent and in lower abdomen as you have mentioned. You could masturbate and the amount of ejaculate was normal.
The possibilities are:
Surely testicular torsion if the pain was on one side and the pain may subside if there is de-rotation but the referred pain in lower abdomen may continue.
Another possibility is of a condition called epididymo-orchitis that is infection of epididymis and testicle, can occur on both sides, usually due to retrograde infection from prostate or urinary tract.
Hence I would advise you the following in such a situation:
Send the urine sample for tests: routine, microscopy, culture and sensitivity.
Tests of blood.
Urgent Ultrasonography of abdomen, particularly for genitourinary tract and of scrotum with color doppler for scrotal contents.
These along with an opinion of a Urologist can give you the proper diagnosis and give a proper plan of further management.
