Red spots after scratching skin lesions can be due to it being complicated with infection or due to the spread of the primary lesion that was itching in the first place. The way the lesions look is very important on the diagnosis. This is why having a picture of the lesion might be very helpful to determine the cause and give proper recommendations.
If the area is getting swollen and painful infection is likely and oral antibiotics may be needed.

Thank you for uploading the picture. Are the lesions opened skin or is it just like hemorrage under the skin. Just to confirm as this is important. Do they hurt? Do you have any fever or any other complaints?
Do you have these lesions only here or in other areas if the skin?

Thank you for the reply and the extra information provided.
If skin lesions are bright red but the skin is intact, then hemorrhage under the skin is considered.
In your case, because the lesions are in a small area in the skin and because you have no fever, no pain or other complaints, you may observe these in the next 3-4 days.
Because of the itchiness that preceded the rash, you may try a steroid cream such as hydrocortisone twice a day in the area.
If you notice no improvement in a few days or if you have any worsening you will need to urgently see a doctor and probably have blood tests to rule out low platelets or other possible conditions (infection, autoimmune conditions, etc)
To conclude:
- because the rash was after local trauma(the itchiness), and it is localizedand not painful and you do not have other symptoms, you may observe the rash in the next few days (you may use local hydrocortisone to reduce local irritation)
- if no i.provement or you have any worseningor new symptoms you will need an urgent visit to rule out important conditions.
