Microcytic anaemia
Detailed Answer:
Hi....
Welcome to "Ask a Doctor"service.
I am Dr Ajeet XXXXXXX
I have gone through your query and reports and here is my opinion.
You have anaemia with a Hb of 9.4.
It has marginally improved since last time.
Peripheral blood film is indicative of Microcytic anaemia. Here ,small size microcytes are formed which do not mature.
Most common cause of it is iron deficiency.
Other causes of microcytic states are anemia of chronic disease, thalassemia, and sideroblastic anemia.
Examination of your stools reveal occult blood. You are having gastrointestinal bleeding and you are losing blood in stools (May be that it is microscopic and you would not be able to notice)
Please share all your symptoms in detail so that I can correlate clinically.
Investigations are always read in context of patient's history and are of limited clinical significance if read in isolation.
Happy living.
Regards.

Microcytic anaemia
Detailed Answer:
Welcome back.
It is good that his diabetes and blood pressure are well controlled on drugs.
He has also modified his diet.
Jaggery, green leafy vegetables, pomegranate and organ meat are good sources of iron.
But that would not suffice.
Iron supplementation in the form of diet, commercially available oral preparations(like livogen),iron injections and even blood transfusion will not help him because of ongoing intestinal bleeding.
The cause of gastrointestinal bleeding should be ascertained.
Treatment of the root cause will only make him retain iron in his body.
Best regards.
