Question: both knees really red and hot to the touch, some pain on the right knee with a little bit more redness. Pain is on the outside of the knee (meniscus area)

I read your query and seen the attached picture here is my advice.
I think you should get further evaluated in order to find what can be the cause of the swelling, redness and warmness on the knees.
You have done rheumatoid factor test but you may also need to do uric acid levels in blood because it can be related to gout or pseudogout and also a x-ray of the knee or better an MRI to see the conditions of the joint ind of fluid is accumulated there.
Use anti-inflammatory drugs such Advil or naproxen for pain and swelling. Use cold compresses on it to relieve swelling and redness. Keep the knee at rest and elevated.

redness and really warm to the touch with also some stiffness. have had lyme disease in the past. RA factor blood test came back negative when I last had it done.

Having arthritis is common sign of Lyme disease and I believe you are having Lyme arthritis.
It can happen up to two years after being infected by Lyme . I think you may require intravenous antibiotics. Talk to your doctor about taking intravenous antibiotics. In some cases it may last even after intravenous antibiotics.

another thing is it’s the worst at night, sometimes it’s not even there at all. It seems like it kind of comes and goes but it seems like an every night thing.
