Hi there
Thanks for the query
Do you have any back pain?
Since when we you having this problem?
Usually exercise intolerance could be heart related or lungs related.
If the EKG has come normal, you can get an ECHO of the heart to rule out any hypertrophy or other issues.
High blood pressure can also be a cause which can be missed.
I hope I have answered your query.
I will be available to answer your follow up queries.

I've had this issue ever since my spine surgery, I wonder if it could be muscular in nature, does this sound like angina? he discomfort can be felt with slight movements, it's only made worse with exerscise . I burp and fart alot abdominal ct shows constipation. This exersicse intolerence has gone on for about two years. It hasent gotten worse, just about the same level. BP is usually 135/85

Angina Would more likely cause pain in the chest that can radiate to the left arm but the jar and the back of the head.
Since you suffer from flatulence it is possible that you have indigestion with gastritis there could also be a possibility of an irritable bowel syndrome.
Have you been ruled out of cholecystitis or gallbladder problems?
You should also be tested for gluten sensitivity as this can irritate bowels and cause flatulence. Also lactose intolerance can cause the same.
Include 3 litres of water daily everyday in your diet. Avoid wheat and fatty or oily foods. Also coffee must be avoided. These also are common causes of flatulence and indigestion.

I've been ruled out of gallbladder issueshad an ultrasound of pancress and liver. Ever since my spine surgery I get tired when walking and feel slightly shorter breathe. When I lay down I get a sore feeling that radiates from my mid back to the front of my stomach and sternum. It's been this way ever since back surgery.
If this were to be angina, can I reverse plaque in my arteries by diet and lifestyle? I had no psychical therapy after my spine surgery and I spend most days sitting or laying I stand about 2 hours a day and walk 30 minutes a day at 10 minute intervals

Usual after Fusion spinal surgery there will be some degeneration of the discs at higher level which is why you may be experiencing some stiffness in the mid back region.
Physiotherapy is extremely important in regaining the flexibility of the spine especially after spinal surgery which is something that you need to consider.
If there are plaques in your arteries you first need to get an angiogram to find out how severe it is and if it can be reversed with conservative measures like diet and lifestyle because in some cases you may need to do certain procedures to help get rid of the plaques. Also Exercise stress testing will help along with ECHO study of the heart.

Since you have pain in the mid back it's after a T5-L2 fusion surgery so we need to rule out any fusion problems.
Your symptoms are more suggestive of indigestion and gastritis rather than angina but even with the normal ECG there is a possibility of exercise induced angina which needs to be revealed by an exercise stress testing or treadmill test.
Hence in order to rule out one of these problems you need to do a CT scan of the thoracolumbar spine and an exercise stress test or treadmill test for the heart. It is very difficult to tell which one it is by just the history which you have provided.

Detailed Answer:
If the backpain is present since the day of surgery it could indicate muscular problem although the pain should subside with physiotherapy.
A normal coronary calcium CT indicates no atherosclerotic plaques but be aware that angina can also occur with spasm or heart arteries without a plaque. If you think you have shortness of breath based on activity with chest pain it is best to do the exercise stress test to rule out spasm of the arteries as a cause of the pain.

if you feel radiating pain from the back to the chest and the abdomen then there is a possibility of nerve compression after the surgery. Also fusion can lead to decreased flexibility of the spine which can lead to pain when you try to burp or take a deep breath or twist your body.
There are some studies which show that diet and exercise can reverse soft plaques. Omega 3, HIIT exercises and diet low in fats and sugars will help.
