Hello I Went To My Gyno For Late Periods. I

Question: hello I went to my gyno for late periods. I am 43 yo not pregnant. she did ultrasound and I have cysts, but not the "string of pearls" found typically with PCOS. she took lab work and my a1c was fine, testosterone was normal, thyroid was normal, prolactin was normal, and fsh was normal. BUT DHEA-s was elevated so she wants me to go to an endroclonigist, which I cant get into till September.
I am nervous.
what kinds of things will the endrocrinlogist do ?
Could this be stress related too ? I am extrememly under stress and anxious.

Brief Answer:
About high DHEAS;
Detailed Answer:
Hello and thank you for asking!
I have gone carefully through your concern.
The interpretation of DHEAs depends on it's result.
If it is over 700, adrenal tumors should be excluded, otherwise it may be due to ovarian cysts.
As testosterone is normal, there's no need to get concerned.
Please attach the results of the hormonal tests for a more detailed answer.
Best wishes,
Dr.Mirjeta

my ultrasound result : Anteverted UT with Normal Contour
No Fibroids noted at this time
EML=6.66mm
Csec scars 11mm and 14mm
RT simple cyst 2.85 x 2.60 x 1.92 cm and complex cyst
(appears Hemorrhagic) 3.03 x 1.53 x 1.89 cm
LT ovary Unremarkable
No FF or adnexal masses noted at this time
my DHEA was 262 and they said it should be under 240
Symptoms I have are late periods, facial hair, mild dandruff, I have gained a lot of weight in the last 6 months.
I guess I am concerned as to why she is sending me to an endocrinologist.

Brief Answer:
About high DHEAS;
Detailed Answer:
Hi again,
What an endocrinologist excludes in these cases is Cushing syndrome, that means an overproduction of cortisol level that causes weight gain, irregular menstrual cycle, and high DHEAs.
S/he may ask you to check cortisol level in the urine and a low dose (1 mg) Dexamethasone test, to confirm/ rule out Cushing syndrome.
Wish you good health.
Best wishes,
Dr.Mirjeta

Brief Answer:

Detailed Answer:
Hi again,
If Cushing syndrome is ruled out and it persists high after a couple of months, then it is due to PCOS.
If it is back to normal, then it is stress related.
