Brief Answer:
This is inflamation and probably a rheumatologic diagnosis

Detailed Answer:
Hello and welcome in Ask the Doctor

I can understand your concern and I am sorry for your situation

The fact that only steroids help you and also the fact that you are taking plaquenil means that a general inflamation is the cause. Generally rheumatologic problems are the main cause. It depends on your diagnosis for example if you are suffering from sclerodermia all your concerns are related to it and it's normal. So I suggest you to talk again with the specialist who have prescribe you plaquenil to explain the exact diagnosis you are suffering and all the concenquences that you might encounter.I think this will relieve a lot your life.
Hope to have been helpful
