Question: hello dr...m having itch in and around scrotum for more than 3 years by now. have tried all medicines like anti fungal cream , fluconazole, itraconazole, fluticasone propionate, retin a gel, antibiotic creams. symptoms are under control during medication and when medicines are stopped then itching aggrevates. m seeing small bumps that causes itching in scrotum and it is some thing like tingling sensation. can you pls advise if i need to do a blood test or some tests in order to find the root cause. also pls suggest if any non steroid medicines that can be used long term.

I have gone through your query and reviewed the attached photographs.
It seems to be lichen simplex chronicus of scrotum.
It is a chronic type of eczema which shows remissions and relapses.
I would recommend you to get your serum IgE levels and fasting blood sugar levels checked.
Meanwhile take antihistaminic like tablet hydroxyzine 10 mg twice daily and apply moisturizing cream containing white soft paraffin like moisturex soft cream 2-3 times on the pubic region.
On the affected area apply calcineurin inhibitor like tacrolimus 0.1 percent ointment once daily at night in minute amounts.
This will improve your condition.
These are prescription drugs and should be taken under the supervision of the doctor.

Thank you dr...m using moisturex from yesterday and also have fixed an appointment tomorrow with the dermatologist as per you advise. consulted again gp yesterday who is treating me for the past 1 month and he said the itching is because of hair follicules and have advised me to continue using retina gel. m bit confused. anyhow have shared you the clear images to your mail id again for your suggestion. will confirm you once i visit the dermatilogist in person tmrw.
