Question: hi, I want to give you all the background then the question. I am 58 and a female. I had a very bad cold last month starting on May 13 that started with white on my tonsils and cough. I went to the PA and had a test to make sure it wasn't strep and it was not. but she did put me on amoxicillin just in case. A few days later it went to my chest and ears. along with the cough. I have been taking Clairton and flownos and I still have plugged ears. she checked my ears last week and said they were clear. This was background on the cold. Please know I do not like to go to DRs and rarely do except the last few weeks I have gone quite a bit, but then last Tuesday I started to have lower back pain on my left side. it felt just like when I had a kidney stone a few years ago. so then on last Wednesday the pain went in my groin area when I walk. I went to the PA again and I had an x-ray taken that showed I did not have any sign of stones on the left side, I did on right side but she said that wouldn't have made any difference. She also checked my urine and no blood. Last Thursday night I have had a vulvar cyst for probable about 2 years, but that night it got absessed and broke. I have another PA apt tomorrow. I sound like a hypochondriac but I am far from it. So here is the question. I have pain that moved from a very painful lower back to a dull constant pain in my buttock, and then when I walk I had a sharp pain in my groin to now a sharp pain up and down my upper thigh. what information can you help me with to get a correct diagnosis. I do not know if the cold or the abscessed cyst have anything to do with the pain, but I wanted you to have some background. Last week the PA thought maybe that it was sciatic nerve so she had me lay down and lift my left leg. I had no pain when lifting but did when I was putting my leg back down and she mentioned that typically if it was sciatic nerve, I would have pain when you lift the leg, not when I put my leg down and the pain was in my groin and lower back at that time. it has never was in the back of my thigh. the pain is so bad when I walk that I feel nauseous. I guess I need to know what you think. thank you for your time. Also I did not have a fever last Wednesday.

You had an upper respiratory tract infection which well responded to an antibiotic course.
Coryzal symptoms ,cough and ear plugging can easily be controlled by anti histaminic drugs and steam inhalation.

Your urinalysis report and X RAY-KUB are normal,that rules out urine infection and calculus.
An x ray is an investigation of choice for stones as usg has poor reach to uterus.
CT is procured if there is diagnostic dilemma and if hydronephrotic changes due to back pressure are to be ruled out.

No one can label you a hypochondriac till all organic causes have been ruled out.It would be unethical for a clinician.

Your ruptured vulval cyst needs an antibiotic course,preferably a combination of amoxyclav and moxifloxacin.
NSAID like Advil would relieve the pain and inflammation.
If whole of the pus does not get drained out,it may need an incision,after the antibiotic treatment when it becomes fluctuent.

Your groin pain and upper thigh pain does not go with the diagnosis of a vulval cyst.
It might be a Groin strain (pulled groin muscle) which occurs as a result of a wrong posture,unknown or known injury or awkward movement of the hip joint,which leads to stretching or tearing of the inner thigh muscles.Pain might be accompanied by thigh muscle spasms.
A magnetic resonance imaging (MRI) is diagnostic and evaluates the soft tissues around the hip joint.It shows muscles, tendons, ligaments, and acetabulum to help determine the source of groin pain issues.
An MRI can also rule out nerve issues, like a pinched nerve in the back that is referring pain to the groin.
Sciatica in L3 might also refer pain to the front of the thigh but that has been ruled out by your physical examination.
Sometimes, lateral femoral cutaneous nerve becomes trapped and cause groin and upper thigh pain.
Muscle sprains and strains are quite common as there are a large number of muscles in the upper thigh and pain in this area.Muscle injury may be in the form of muscle sprains or muscle strain.
Sprains and strains can affect any of the many muscles, ligaments, and tendons in the thigh.
A sprain is a torn or overstretched ligament.
A strain is a torn or overstretched muscle or it's tendon.
This requires hot fomentation,analgesics,muscle relaxants like tizadine and physical therapy.
In addition to simple analgesics,sometimes drugs like amytriptaline and gabapentine are required
