Hi XXXXXXX
Welcome to Ask A Doctor service
I understand your concern
Fever along with painful micturition might suggest urinary tract infection(UTI) that needs antibiotic treatment. But for a better picture, kindly give me few more details:
1. His age
2. When did fever and crying on urination started?
3. Is the crying present throughout urination, or just at the beginning only?
4. Is there an increase in urine frequency?
5. What all medicines he is taking?
Kindly get back with details, so that I can guide you better

Brief Answer:
Get urine tested
Detailed Answer:
Hi Santhosh,
Thank you for the details given.
From the given details, he is probably having a UTI as I mentioned before. Now we have to confirm this by doing urine testing. Kindly get his URE(urine routine examination) and urine culture tests done.
URE report will be available instantly and we can start empirical treatment based on this. A urine culture report will be available in 48-72 hours and will help us to decide regarding the final course of treatment.
If he is otherwise playful and feeding well, oral antibiotics for 7-10 days will suffice. If looking dull, not taking feeds or recurrent vomiting, we will need admission and injectable antibiotics.
Also, if confirmed we will have to get an ultrasound abdomen done to assess the urinary tract.
Kindly get back with the reports or discuss with your pediatrician
