However, the right shoulder seems to be injured (there's labral tear). The ac joint arthritis with subchondral marrow oedema could be secondary to this injury only...
1 year is a long time with this kind of shoulder injury. Hence it's strongly recommended that he visits an Orthopaedician for an immediate review...Ideally, it should be surgically repaired and corrected for better healing...
In the meanwhile, he can take a mild painkiller as ACETAMINOPHEN/PARACETAMOL 650mg thrice daily after some food...Avoid any kind of heavyweight bearing on the right side...Please keep the shoulder rested, a shoulder immobilizer can be used to keep it in a fixed place for the time being...
