I Am 64 Year Old Woman. For Over 15 Years
Is this part of AF? Or do you feel this is another heart problem.
Could I still take flecainide to get my heart back to normal beats?
What are other test than eco and ecg need to be performed to find out if there is another underlying problem?
Some extra informations:
I had echocardiogram and stress test 7 months ego all was normal!
Plus the medications mentioned before, I also take carbimazole 5mg for over active thyroid and Crestor 10mg for high cholesterol.
Also some supplements: taurine 500
Magnesium
My recent blood test all normal except last few test shows slight drop in white cell count. My General Practitioner feels this might be caused by carbimazole .

Hello!
Welcome to Ask a Doctor service!
I passed carefully through your question and would explain that the flutter that you are feeling could be related to paroxysmal atrial fibrillation.
The pulsoxymeter is not the best way to measure your heart rate, as it depends on the blood flow arriving in the toes. So it is not a very accurate way to monitor your heart rate.
As you have been suffering from paroxysmal atrial fibrillation in the past, it is very probable that this is happening now too.
Coming to this point, I would recommend performing an ambulatory 48 hours ECG Holter monitoring in order to examine your heart rhythm trends for a prolonged time.
If there is presence of paroxysmal atrial fibrillation, flecainide should be started again. But, you should continue taking it for a long time in order to prevent these episodes of atrial fibrillation.
The thyroid gland dysfunction is an important factor influencing in the heart rate. For this reason, I would recommend checking your thyroid hormone levels, if you have not already checked them.
It is Ok to take the supplements that you are taking (magnesium and taurine), as the doses are normal.
Hope you will find this answer helpful!
I remain at your disposal for any further questions whenever you need!

Dr. Ilir Sharka, Cardiologist

few more points and clarifications
i do realize that the pulsorymeter not most accurate. probably the same with the fitbit watch i wear that has heart rate. but couple of times i tested these against my General practioner%E2%80%99s and emergency crew readings and they corresponded well with theirs. unless as when i have the episodes as my pulse is irregular these can not get proper reading. never the less before on my episodes these were showing an increase to heartbeat, now they show quite a drop.
i can see your point on 48 hours monitor. the only problem is that my episodes are quite irregular. at times nothing for month or 2, then suddendly happens few within one week, or at other times happens just one time and nothing again for few months.
my thyroid function at this point is quite controled with medications. so should be the cause at the moment.
and the last point isthat i have never been on flecainide on regular base. only when i asked the cardiologist if there was anything i could, so i didnt end up to emergency, she gave e this medication and ask me to take it only and if i developed those episodes.
now my questions:
not counting the 48 hours monitor, is there any other way to get answer of what is happening ex xray, MRI, ...?
should i ask to be on flecainide when though my episodes are regular?
also couple of cardiologist i saw were having different view of either i should go on blood thinner or not. what is your view.?
of course i take asprin but this is apparently not enough.
my last question is that my episodes are happening at times, when my stomach also not good. can you see any relevant of this with digestive system?
thank you

Detailed Answer:
Hello again!
Thank you for the additional information!
As you refer, these episodes could be bradycardia. In such case an anti-arrhythmic drug such as flecainide should be used with caution.
I would like to directly review your cardiac ultrasound and stress test report if you could upload it for a second opinion, because there are some parameters in these tests which can help orientate toward the possible cause (a possible cause of bradycardia would be sick sinus syndrome).
Regarding your stomach, it is true that it may influence in cardiac arrhythmia. You should know that gastro-esophageal reflux may trigger cardiac arrhythmia. For this reason, I would recommend trying an antiacid or PPI (omeprazole, pantoprazole, etc.).
Regarding the blood thinner, as long as you do not suffer from any other disorders like arterial hypertension, diabetes, etc.. aspirin is quite enough to protect you from any future stroke.
Hope you will find this information helpful!
I remain at your disposal for any further questions or whenever you will be able to upload those test reports.
Kind regards,
Dr. Iliri

Could please advice me with the followings:
1- my normal heart rate has been a bit high. My resting heart beat is usually between 75 to 95 (almost if I don%E2%80%99t do anything) sitting, watching tv, reading. Goes to 100 if seated but am typing and have some hand activities. In time of sleep or complete rest, according to my smart watch it could go to 62. At other times as said before is usually high.
( since first time my condition was known, they put me on Metoprolol. Half day half night. The cardiologist followed me was not too keen for me to continue, but as the medication was controlling my occasional extra beat. And was helping me to feel calmer she ask me to take quarter morning and night. But even the quarter would drop the rate form 85 to 65..)
So now because of those episodes of heart rate drop, with the knowledge of my cardiologist I am off Metoprolol.
My question is this: is my heart rate of 75 to 95 is too high? Does it justify to go back to this medication?
2. My second question is what is the acceptable and safe heart rate while I do normal activities? Just dishes, cleaning cooking? Basically how high is too high?
3. Can it help if I start cardio friendly exercises? What should be the targeted heart rate to be safe?
In addition: While I was writing these my heart beat started to go up 105 110 up to 120 then Came down 93 then dropped at 50 and then went up again quite quick to 105 .. then went down to 98 and in the past 20 minutes is just around 93 to 95. Was this a typical AF?
thank you so much. i real apreciate to be able to consult with a cardiologist in this manner.
warm regars

Detailed Answer:
Hello again!
I understand your concern and would explain that your heart rate fluctuations are quite normal during different activities, depending on the type of physical activity (it is normal to have a lower heart rate while resting or sleeping and a higher heart rate during anxiety or physical activity).
The heart rate 62 is not too low.
Your mean heart rate is quite normal and doesn't justify the need for beta-blockers. So, I would not recommend taking any medications for there heart rate fluctuation.
Regarding cardio exercises, I would recommend starting slowly and increasing gradually the workload.
It is Ok to reach a heart rate of up to 150bpm during physical activity. It does not harmful to your heart.
The last episode of heart rate fluctuation that you describe is not typical of atrial fibrillation. Anyway, the watch application can monitor the heart rate but not the heart rhythm.
The fact that these episodes are shortly after eating could be indicative of acid reflux.
A loop recorder would be a more accurate tool to examine the heart rate during these episodes.
Hope to have been helpful to you!
If you have any other questions, please don't hesitate to ask me again!
Wishing all the best,
Dr.Iliri

I understand this is not show exactly what was happening, just also the highest and lowest point happened in shorter periods of time and as said shortly after food.
My question:
AF is associated only with heart beating high or also include drop in heart beat?
If not, does this sudden drop in heart shows another underlying problem?
Should I go to emergency room with this? or is it ok to stay home if I do not have any other symptoms.?
I am having at the moment a bad indigestion caused by gastritis I was diagnosed with. I have the feeling any time my stomach is bad my heart episodes also happen. Do you see any relevancy?
With this new image do you think I should go through more tests?
Thank so much for your help and advices
Warmest regards

Detailed Answer:
Hello again, dear Yasmin!
Switching to an Apple application seems to be a rationale choice. It has been a wide trend recently. It may detect more precisely your heart rate fluctuations and event give some clues to any possible arrhythmia.
Regarding your digestive symptomatology, I would explain that gastritis, acid reflux or hiatal hernia may be associated with palpitations, premature heart contractions due to local heart compression or neural irritation. May be the right strategy would be starting a gastric anti-secretory drug (such as pantoprazole, omeprazole, etc.) to decrease any potential digestive influence to you heart rate activity.
Meanwhile, I find your heart rate fluctuations quite normal, they are proportional to everyday physical activity. You shouldn't worry about it!
Atrial fibrillation is characterized by an irregularly irregular rhythm, not by day-nigh heart rate fluctuation. So nothing specific for atrial fibrillation is indicated by your application recordings.
In case you don't experience palpitations or other disturbing symptoms you don't have to be concerned; otherwise if palpitations become frequent a medical consultation to monitor your heart rhythm with more professional devices is required.
That's my opinion.
Hope it could help!
Let me know in case of any other questions.
