Question: i am a 51 year old male with no history of colon cancer in my family on either side, but both ny parents had chronic prolapsed hemorrhoids. I%E2%80%99ve never been diagnosed with them, but i too have grade 4 prolapsed hemorrhoids, because i can feel them all the time. these seem to bleed a lot. sometimes i see bloody mucus or blood clots on the toilet paper and the blood drips into the toilet. is this most likely the hemorrhoids doing this or can it be cancer? i have OCD and am scared of cancer very much.

I have read your query and here is my advise.

* So far as colon cancer is concerned, there are multiple factors responsible
for progression of condition as genetic, dietary, life style and others.

* Coming to your symptoms of possible hemorrhoids and scarring of cancer,
let me explain details of both in a nut shell.

@ Cancer colon present in various clinical set up depending upon the site,
size and severity of the condition.

Right sided colon cancers as of ascending colon, present with more of bleeding, exophytic mass, iron deficiency anemia, weight loss in majority of the cases.

Left sided colon cancers as sigmoid colon, rectum, descending colon present with partial obstruction, alternate constipation, diarrhea, colicky pain with hematochezia.

These are usual and gross presentation in general , there can be individual differences.

* Coming to piles, that also present with different symptoms depending
upon the grade.

@ 1st degree piles are asymptomatic and found accidentally during
examination.
@ 2nd degree and 3rd degree piles do present with varying discomfort,
occasional bleeding per rectum, with staining of stool , prolapse and goes
inside on its own during act of defecation or straining.
There is no severe anemia unless continuous ongoing bleeding.
@ 4th degree of piles are prolapsed outside and never go inside even after
you try anyway and it is a surgical emergency where surgery is must.

So far as your symptoms are concerned, I quote more possibility of hemorrhoids of 3rd degree most likely rather than cancer.

Still it is recommended to get one time physical examination with per rectal and proctoscopy with your consultant surgeon for the final diagnosis and best management.

Till that time, best thing to do is

- avoid constipation
- use bulk forming laxative as ispaghula husk or else
- drink more liquids
- consume high fiber diet
- avoid all spices, non veg till possible
- avoid stress and anxiety
- sitz bath ( sitting in tub filled with lukewarm water 10 minutes each 2-3
times a day )
- avoid prolong sitting on hard surfaces, long drives

so these hemorrhoids can bleed a lot like this and cause blood on the paper and some clots? These hemorrhoids are very large and seem to be prolapsed all the time.

Welcome for follow up.

* The permanent mass seen outside is usually external hemorrhoid and does
not bleed by enlarge.
It is the internal hemorrhoids which bleed more and giving loss of blood.
The prolapsed internal hemorrhoids as told in stage 4 are usually
thrombosed and do not bleed but gives intense discomfort of sitting.

However, if you can provide a photo picture ( if no objection ) showing the
external mass, I can comment further in this direction as well.
