Question: i asked a question 2 days ago and still have not a response to the question regarding my sister XXXXXXX recently having her right leg removed due to a bacterial infection from complications of her diabetes my sister is now in a Rehab facility and in grave condition simple due to being in excruciating pain and no pain management she also can no longer endure the dialysis that she was taking 3 times or 4 times a week since her amputation she says it hurts to bad to go through dialysis and refuses dialysis treatment so with that being said the hospital is now deeming her in Non compliance and is leaving her to her own demise death is slowly approaching I need help i did some research as to why the dialysis treatment is now so painful to her and it could be one reason that another minor surgery may be due to readjust the "Shunt" that is causing blockage or something and the blood flow is not circulating properly they are neglecting my sister and I"m here in California my sister XXXXXXX is in Irvington New XXXXXXX my hands are tied whereas if I could just get her through this hurl of getting back on Dialysis than she could maybe get enough strength to go on to fight for her life!!!! she so very weak she says and she's swelling up from lack of dialysis treatment. I'm a praying women and my prayers are that an immediate intercession for my sister takes place and hospital staff and Administration finds some type of ethics and respect for human life and not pick and choose whom they feel is not complying so therefore just leave them to a slow agonizing death!!!! that's my question how can I get help for my sister like Right now?

Brief Answer:
you must discuss with her treating doctors at her hospital.

Detailed Answer:
Hello and welcome to `Ask A Doctor` service.
I have reviewed your query and here is my advise.

* As far as the narration of the issue what has been provided, the
condition of your sister is quite critical and she is fighting with
underlying issue of a detrimental level.

* So far as dialysis is concerned, the blockage of shunt and other vital
parameters may be not sufficient to allow the doctors to proceed for
the same.
Any consultant on the globe will try his or her best as per the
knowledge and availability of infra structure for the particular case.
Lot many things depend upon the patient part as well in terms of
getting the desired results.

* Yes, the pain must be managed in any manner by the consultant with
various options available and best suitable for patient ( the shouting or
scolding the patient is never allowed in any XXXXXXX of the world ).

I would rather suggest you to discuss the case in details with her consultants by visiting the hospital and if needed shift her to a higher
equipped hospital than that centre.

Wish her fine health ahead.
God bless her.

I'm not clear on the blockage of the shunt may not be sufficient to allow the doctors to proceed for the same what does this mean? I can't see why it may not be sufficient for the Doctors to do the same? Bare in mine my sister has been doing dialysis 3 to 4 times a week prior to her amputation now and several and I mean several dialysis patients refuse dialysis treatment due to the pain of that treatment so my sister is not doing anything out of the ordinary nor is she being in-compliant she can endure a certain level of pain simply because she had natural childbirth with 5 children so i'm not clear on why the surgeons or Doctors can't go back into the Shunt site and see if that is the root of the problem I am seeking a solution not engaging in the problem why is it not sufficient to do this minor procedure in her arm re-adjusting the Shunt?

Brief Answer:
follow up.

Detailed Answer:
Hi
Thanks for the follow up.

The blocked shunt can be re created, but I think the general condition of the patient is not
supportive for proceeding even for that too can be a reason ; because in that instance also
there are chances that patient goes in shock.

But for a critically ill patient, it is worth to take that risk too in my opinion.

Regards.
