I Took An Unkown Substance, Purportedly Oxcodone. However, I Doubt

Question: i took an unkown substance, purportedly oxcodone. however, i doubt that it was. it has been 80 hours post ingestion. after ingestion my stool changed to diarrhea and was yellow in color with vegetables being undigested. today, it seems i am slightly constipated and having some trouble evacuating my bowels. the color has gotten darker but is still a very dark yellow or light brown, but it has gotten to a normal consistency. 30 hoursi visited a doctor as i had stomach pain, the urgent care said i should be ok and nothing required immediate attention. she stated to get bloodwork done by a primary care when i had a chance and my liver was not inflamed. what should my course of action be?

Brief Answer:
Hard to say
Detailed Answer:
Hello, I'm Dr. Branch, thanks for using Ask a Doctor. It's a little hard to say what should be done at this point. Your symptoms are concerning for a possible liver problem, but if the urgent care said the liver was not inflamed, then it sounds like they already checked labs looking at your liver. I am not sure what bloodwork they would want your primary care to do. Are you still having any stomach pain? Are your stools normal now? Can you send a picture of the labs that were done at the urgent care? If you are able to give more information hopefully I can give more specific advise.

Sorry for not being clear. I have not had any labwork done as of yet. My last bloodwork was done two weeks ago pre ingestion. That came back fine. That is due to accutane which i am taking. She just felt my liver area to see if anything was happening there. My stools are coming out as long thin strips and, while it has hardened, is still not normal. I have had a slight urge to throw up for the past few days but the one time i almost did it felt like acid reflux. I ate so much greasy food yesterday. I know it spiked my blood pressure.

I am probably not being specific enough. The nurse practitioner felt my liver area and did not see any reason to go the er based upon physical examination. I am straining somewhat to go to pass stool and it is coming out slightly, as thing strips. I primarily have gas.

Brief Answer:
Follow up with your regular doctor
Detailed Answer:
Ok, I see. I am surprised they didn't check any labs. It sounds like you are going to be alright, I am not sure what you ingested, but it sounds like it was either toxic, or perhaps you just happened to catch a stomach bug around the same time which gave you diarrhea. It sounds like you are recovering, though, so that is a good sign. I would just follow up with your regular doctor sometimes in the next week or so, and talk with them about checking labs again just to make sure everything looks alright. The main reason for this would be the change in color of your stool, which if it was very pale or yellow, could be a sign of a liver problem. However, without being able to see it for myself, it may be normal and nothing to worry about.
I hope that helps, please let me know if you have any questions about any of that, and I would be glad to discuss it with you further.

Brief Answer:
Signs to look out for
Detailed Answer:
If you noticed your eyes were yellow, or you were having nausea and vomiting so much that you couldn't keep anything down, or severe abdominal pain, those would be reasons you should go to the ER for. Otherwise, it doesn't sound like you need to go to the ER and can just wait to follow up with your doctor in the clinic.

I am fairly certain whatever I ingested was toxic and I am going to get help for my substance issues. I did eat pretty bad that day as well so my thoughts are the same as yours, in that it could just be a bug. The color of my stool was initially dark yellow and now it seems to be getting darker. It is not pale or light yellow. I am definitely going to go in and get bloodwork. I am just making sure nothing requires an er visit right now. What symptoms would be to look out for?

Thank you, should I lay off on the Accutane until then?

Brief Answer:
You can continue Accutane
Detailed Answer:
If you are feeling alright now, I think it would be fine to continue the Accutane. Only if you start feeling worse or start having any of the symptoms I described before would I stop taking it and get in to see your doctor or go to the ER.
