
Is This Just Pile Or Is It An Abcess? Im

Palpation of the site has to be done by a healthcareworker. No one can differenciate pile from abscess unless visible pus drainage is there just by watching a photo. Anusol won't help.

I have reviewed your query and here is my advice.
From the photo attached by you it looks like a pile but to differenciate it from abscess a health care worker has to touch the area has to note down temperature.When ever there is abscess in lay mans language a war is going on between bacteria's and our blood cells therefore circulation to that area would be increased to supply more and more blood cells causing raised temperature,Formation of pus may lead to chills with rigors.However if you can't go to a surgeon right now touch the boil and feel if it is hard(alarming) or a bit soft,Secondly note that its feeling hot or is near body temperature.
Have you noticed any white or yellow discharge in the surrounding area?
So just by seeing a picture its difficult to say if its just pile or abscess is developing.If you are in too much pain then most likely it can be abscess(developing or developed) which needs to be drained by a surgeon.
Oral Antibiotics won't help as they don't work once pus formation is there.
As of now i would suggest you to take a pain killer like ibuprofen(Over counter) one dose to provide some relief in pain and after that visit the nearest emergency.

Firstly you should go to doctor however if you want to give a try suggestion is,
Take antibiotic Zithromax 500mg(Azithromycin) once daily with Ibuprofen one tablet twice daily along with seratiopeptidase 10 mg thrice daily(all of them are not over the counter except ibuprofen)
If still not relieved go to doctor asap.
