Question: both the kidneys show irregular outline and mild increase in cortical echogenicity-cortical scarring with mild nephropathy. Tiny echogenic shadows are seen scattered on both side

The ultrasound report mentioned shows that both the kidneys are affected and are not working properly. The reason and the extent of damage is not interpreted from an ultrasound report.
I need to know following things-
1. Is the patient having Diabetes or hypertension.
2. What is serum creatinine levels, serum calcium and phosphorus levels.
3. Urine routine examination findings,
4. Is there history of repeated urine infections.
Kindly provide with above information so that I can guide you regarding the specific cause and extent of damage. Any other questions in your mind can also be stated.

Your reports show that there is some disease in the body that is causing high calcium levels. This needs investigation to narrow down the cause of high calcium levels. Due to this high calcium level it is getting deposited in the kidney and causing early kidney dysfunction. There is probably a urine infection also which can be confirmed by urine culture and treated.
High calcium levels can occur due to
1. Hyperparathyroid disease. Get a TSH test done.
2. Malignancy, tumor, myeloma - group of diseases which needs testing.
3. Overdose of medication.
