Question: my 7 year old daughter has been complaining of stomach spasm since 3 days,she doesnt have any fever.She has been eating well and is active.But today she came back from school early as she complained of pain,what should be the line of action?

Noted the history about your daughter.
The commonest causes of pain in abdomen at the age of 7 are as follows:
Worms.
Tummy tonsils means abdominal lymph node enlargement.
Appendicular or colon fecoliths.
I would advise you the following in such a situation:
Consult a Pediatric or General Surgeon/Pediatrician. This allows for proper examination and clinical evaluation which is an important step. Proper palpation of abdomen gives lots of information.
To be supported by High Resolution Ultrasonography of abdomen.
Tests of urine, stool and blood.
All these will give the most appropriate diagnosis and a chance of proper management as per the exact cause found.

i went to the doctor,he has asked us to wait,gave us meftal spas and lanzol.although she still is complaining of pain,i noticed at night she was clenching her teeth,could she be having worms?
i have also notice her urge to scratch.How can we be sure about this,Is zentel safe to give right now?
also should we go for an ultrasound abdomen or wait!we also got a routine urine test done,which is normal.

I have mentioned the possible causes above and needs the investigations as written to get a proper diagnosis.
Night scratching or anal area is known to occur in thread-worm infestation. And better be diagnosed by stool test. Zentel be given after the advise of the treating Doctor and investigations. Mixed infestation can cause problems like intestinal obstruction.
Please insist on all the tests as mentioned to get a proper diagnosis.
Continue the medications as already advised.

doctor i have attached the two reports,the report for urine is also normal.But she is still complaining about pain,what should we do.she has been havinh meftal spas and has had 30 ml in 4 days,her weight is 25 kgs.I guess i told you she had been clenching her teeth while sleeping,yesterday and i gave her zentel,how much time does Zentel take to effect.
buy plz suggest the further line of action,should we give her antoboitic for food posioning?

Zentel takes about 1 - 3 days for action. You have to watch the stool for worms and also send stool samples for 2 more days.
Stool is not showing any infection that is suggestive of food poisoning.
Where exactly is the pain situated?
Any nausea, vomiting or bloating?
Are the stools soft, loose or hard?
Any fever and chills?
Any relation of pain with food, passing stool or urine?
Is the pain continuous, spasmodic (comes and goes), mild/moderate/ severe?
Please give detailed history.
Which Specialist have you consulted?

The Ultrasound shows multiple internal echoes in urinary bladder so send the urine for: routine, microscopy, culture and sensitivity too. It also shows small mesenteric lymph nodes which can cause pain.

thank you for the response.will monitor stool no problem,the Pain is below the navel area but all over,no nausea,vomiting or bloating.But i guess she complains more after food,although its there otherwise also,she has come back from
school twice bevause of pain but she looks ok when back.the stool is hard when she begins and then turns soft.No fever or chills.yes pain is more about meal.the pain is spasmodic,and moderate or mild.we met a pediatrician.

Since there is no nausea, vomiting and bloating the possibility of intestinal pathology are less.
Hence the cause can be related to urinary bladder or lymph node enlargement, there is no fever or chills too.
If there is no relief take an opinion of a Pediatric Surgeon as an actual abdominal examination also helps a lot for clinical diagnosis for the cause of pain and guides for any further investigations for proper guided management.
