The medications used to treat pinworms include albendazole, mebendazole, and pyrantel pamoate. Of these, pyrantel pamoate would be preferred for you, since it does not have much systemic absorption and would not be likely to get into your breast milk.
However, if you only had the Albendazole or Mebendazole, while it would get into your breast milk, studies have not shown any harm to babies, especially at 21 months, with you only partially breastfeeding, I don't believe it would be enough to cause any harm and you could take those medications as well.
