Hello doctor, I am XXXXXXX Bhardwaj and here are answers to your questions: 1. I don’t know the exact reason. But I have been a very bright student always but unfortunately I failed in my first year of Economics hons. After that, I pursued Company secretary course ( which was very difficult to pass) but i worked really hard and now i have completed it last year. But since 2-3 years I have been feeling depressed on and off. Also, due to transfers of my father’s job we shifted from XXXXXXX to XXXXXXX then to XXXXXXX and to XXXXXXX in 2019 February, i did my intership here also for 6-7 months but was very difficult for me bcs of language problem.But now since 1 month, I have been feeling: * Loss of interest * Guilty * Not able and wanting to wake up in morning ( this i have been through 1 year) * Weeping mood * Feeling down and having negative thoughts * Weakness/ low energy whole day * Headaches, heaviness in chest sometimes * Having nightmares sometimes * Concentration problems * Feeling of fear of loss of close people in my life etc. * Sometimes sleeping more and sometimes not able to sleep till 4 am in morning 2. Earlier i used to have these symptoms on and off, but now I have been feeling hopeless whether i will be able to feel normal again 3. I have now completed my internship but I don’t feel like doing job anymore ( I feel like i don’t have interest in this field), i like singing and making music videos but since 9-10 days I am not able to do that also,), i have 2-3 close friends i don’t feel like talking to many people. And to talk on phone with someone feels like a big task for me. 4. I am not clear on what is the query actually however i have been feeling these symptoms on and off as mentioned earlier. 5. No, i took only some ayurvedic medicines. 6. Yes my father had it in 2007 for 2-3 years and my mother for few months in 2019 ( now both are normal) 7. I like to assure you that this is completely treatable disorder and can be manged with either medications or psychotherapy or combination of both.

Since the severity of depression seems in range of moderate depression I feel psychotherapy can be tried first as does not have any form of side effects provided psychologist is available in your city. In case psychologist is not available medications are the default choice.

In similar situations escitalopram 5 mg is my first choice but unfortunately alike any other medication, it is not completely free from side effects. Howver the side effects are limited and most of individuals report little side effects with this dose. I need to acknowledge that 5 mg is just starting dose and we often need higher dose for most patients.

Regarding your last question I feel this is beyond the scope of question and answer format as counselling/psychotherapy needs hours of interaction.

For enhancement of serotonin level there are two molecules with adequate supporting literature and have near zero side effects. One of them is precursor of serotonin and known as 5-HTPn and available inmarket.

The other option is S adenosyl methionine which is also easily available in the market.

In my experience 5HTPn works better for individuals with typical depression.
