Brief Answer:
Infected with chlamydia and with genitourinary tract infection signs.

Detailed Answer:
Hello dear, welcome to Ask a doctor service.
I read your query and here is my advice.

The report says that you are having an STD caused by chlamydia trachomatis and you are having signs of a genitourinary tract infection. Acute means of recent onset. The signs of genitourinary tract infection may include burning and pain during urination, pain during sex, low abdominal pain, increased and yellow vaginal discharge, in males yellow discharge from penis, swollen testicles.
It means that you need treatment immediately.
You may use 1gr of azithromycin or a 7 day course of doxocycline using 100mg twice daily under your doctor's care.

Hope I have answered the question.
Let me know if I can assist you further.
