Brief Answer:
A few investigations need to be done

Detailed Answer:
Thanks for using the Ask a Doctor service.

I have gone carefully through your query and understand your concerns. Most often such fevers are due to viral cause and irrespective of treatment they resolve in a week or so. However, since you mention that chills are present it would be good to get the urine tested for routine microscopy and culture sensitivity since urinary tract infection can give rise to similar complaints. Also get a blood smear checked for malaria parasites as that is another potential cause for fever with chills. If both of these come negative you have no reason to worry.

You can try giving Meftal P which is more effective than ibuprofen. Also give tepid sponging.
