My Friend Who Is A Commercial Airline Pilot Had Sex

Question: my friend who is a commercial airline pilot had sex with an escort almost 3 months ago . he had protected vaginal and unprotected oral . half way into the sex he pulled out an noticed black residue at the end of the condom and ask her what this is and she claimed her periods is approching . he wiped off the residue with a wet wipe and continued . about a month or so after the encounter he got a diarrhoea which lasted for 4days , after a week or so he had sushi and got diarrhoea and stomach craps which lasted 2 days and again he had some food from outside and had diarrhoea , i guess a week after the second diarrhoea , a day after that he noticed 4 small lymph nodes . 3 under the jaw and on the neck , they are in the shape of a pea .. not painful . and maybe around 0.5 centimeters . since this has never happened to before we browsed the internet about it . and read some HIV related topics he started to panic and took leave since he couldnt focus and after that he got a ulcer on his lips and again one on his tongue after 2 weeks ... my question is
1) what are the odds that he has acquired hiv
2) what are the risks of the exposure he had
3) is screen for HIV warrented in his case
4) does this seem like an ARS symptoms

Brief Answer:
The risk of getting HIV is very unlikely.
Detailed Answer:
Dear Zybernex,
Welcome to Ask A Doctor Service.
Your friend had protected vaginal sex with an escort so the risk of getting HIV is very unlikely. Diarrhea, gastric upset or lymphnodes may be due to some other reason but not due to HIV because the symptoms of ARS will take at-least 15 days to develop after the exposure. Consult your physician to isolate the cause and follow the treatment regime. As such no screening is required but if you want to get tested then wait for at-least 15 days for reliable result. For any further query feel free to ask me again.
Thank You

you still think hiv infection is extremely unlikely given that the vaginal was protected and the oral wasnt ?

Brief Answer:
Yes HIV is very unlikely.
Detailed Answer:
Dear Zybernex,
Welcome back and thanks for the follow-up.
Lymphnodes and mouth ulcers are not specific symptoms of ARS or HIV so the risk of getting the infection is unlikely with your exposure. As 3 months had already passed go for 4th generation HIV Duos test and the result will be 100% conclusive because the window period varies from 15 days to 3 months. Calm down, allay the fear or anxiety and get tested. The result will surely come as non reactive.
Thank You

Brief Answer:
The lymphadenopathy is generalized.
Detailed Answer:
Dear Zybernex,
Welcome back once again.
The lymphadenopathy in HIV infection is generalized means the enlarged lymph nodes are found in two or more seperate areas of the body usually on either side of the neck, under the jaw or in the armpits and the groin. Sometimes they might be painful or swollen.
Further, any number of diseases can cause lymphadenopathy from sore throat to life threatening cancer so it is not the sign of the disease rather it is indicating the strong immune system for controlling the infection. Just calm down, go for the test and the result will definitely come in your favor.
