This could be a fracture, ligament/tendon rupture, gout, arthritis, sinovitis etc.
You need further testing after consulting an orthopedic surgeon. The testing may include XRay of the foot, serum uric acid levels, complete blood count, serum calcium and vitamin D levels and other tests.
Meanwhile keep the foot immobile with an ankle brace. It should be kept immobile for atleast 3 weeks in case of sprain or fracture.
