After going through the medical query raised by you I can understand your concern for your wife’s die would like to know the following things before proceeding with any medical opinion

1. Any previous history of heart disease
2. Was angiography done for her anytime in past
3. Diabetes or hypertension
4. Why all these medications she is taking.

I send the my wife medical histoty. Now daily she is suffering with Gastic trouble.so pl. Give any advice or prescribe medicine. From 10 years she is suffering with diabetes. Bp from 4 years. In 2019 angiography had done. Present she useing the tablets as my doctor prescribed, which i attached files. So pl. Give suggestion any changes in her medicine.

Hospital provided C D. they had not provide hard copy. they had given report and CD.

Sir, they must have given some written report of angiogram. It can be on a separate paper or on the discharge summary papers.
