I have reviewed your query.

I need to ask if you are married, trying to conceive, your age, is your flow heavy, any other symptoms like fever etc, any medicine intake, any other medical history apart from PCOS, When was your last menstrual periods before this episode, are your periods regular or irregular?

Regarding prolonged bleeding, it could be due to many reasons. Therefore we should get tested with ultrasound pelvic, complete blood count, thyroid profile, blood pregnancy test, fasting blood sugar, and pap smear.

Medicines like Tranexa may help in stopping bleeding in some cases, however these should not be taken if there are any structural problems like missed abortion, fibroids, etc. Therefore gynecologic visit needed to get evaluated.

Still the cause of prolonged bleeding is not clear. Therefore we should get tested with pelvic ultrasound, complete blood count, thyroid profile, blood pregnancy test, fasting blood sugar, and serum prolactin levels. Atleast after getting ultrasound done and if its clear, we can start tranexa.

PCOS can cause irregular periods and prolonged periods can also happen if the previous periods were more than a month ago or were scant. When was your last ultrasound? If it was months ago, then we can have repeat ultrasound to see for any new development. Otherwise tranexa can be taken. Also if you XXXXXXX bleeding heavily now, then rehydration must be done by drinking lots of fluids and associated anemia should be treated.
