Brief Answer:
I would explain as follows:

Detailed Answer:
Hello,

I understand your concern and would explain that it is important to further investigate on the potential reasons underlying pulmonary hypertension: whether it is due to a chronic pulmonary disease or due to the pulmonary artery disease (pulmonary embolism) or due to a left heart dysfunction.

In this regard, a VQ test would help investigate on a possible pulmonary embolism and also exclude any possible chronic pulmonary disease.

I would also like to directly review your cardiac ultrasound report, if you could upload it as attachment to this question.

Regarding the ectatic descending aorta, I find it reasonable to follow up with a cardiac ultrasound after 1 year, in order to investigate on its possible progression.

Hope you will find this information helpful!

I remain at your disposal in case of further questions whenever you need!

Opinion as follows:

Detailed Answer:
Hello again!

Considering the fact you have normal biventricular function and are quite asymptomatic (clinically normal); a periodic followup 6 month to 1 year seems reasonable.

But, from the other side, it is necessary to investigate any possible underlying cause in case you have confirmed pulmonary hypertension (if it is exactly measured by means of cardiac ultrasound).

In this regard a Ventilation/Perfusion (VQ test) scan of pulmonary angio CT would be helpful. Once any potential lungs disorders is ruled out, then an exact follow up strategy could be determined.

Let me know in case of any further questions.

Regards,

Dr. Iliri
