As per your history, you have diabetes since 18 yrs so the tingling sensation in the legs may be due to diabetic neuropathy.

Do you have history of injury/sprain to back, shooting pain through leg, leg cramps, fatigue, weakness, dizziness?
For further assessment, I would suggest you to consult with physician. He may procure nerve conduction studies, vitamin b 12 levels, MRI back with whole spine screening, Thyroid profile, as per evaluation to rule out other possibilities of vitamin deficiency/pinched nerve/underactive thyroid.
Further treatment mainly depends upon the underlying condition. Till then for pain you can take tablet acetaminophen. Apply ointment containing capsaicin.
Along with that Use well fitted cushioned shoes. Avoid bare foot walking.
Now answer to second question, Regarding yearly health check up plan, as you are diabetic and hypertensive, I would suggest you regular blood sugar levels, blood pressure monitoring, lipid profile, renal function test, foot examination and eye check up sos after physician consultation.
